Connect Home Care Agency provides initial and ongoing home care assessments and tailorized care plans designed to assist our clients and family members with activities of daily living including hygiene care, dressing, mobility, continence, eating, medication reminders, grocery shopping, chores, reading, interpretating and completing of forms, scheduling appointments, appointment preparation, referrals, social companionship, respite for the caregivers, and sitter services. Connect Home Care Agency provides our clients with screenings to identify barriers that are contributing to a reduced quality of life and self-efficacy. In addition, we assist our clients with understanding they’re at home treatment plans provided by healthcare providers. Connect Home Care Agency collaborates with healthcare providers on behalf of our clients by consistently documenting and reporting health-related findings. Our in-home aide care, respite, companionship, and sitter services are overseen by the agency Director and staff Registered Nurse.
